This is the first of a two part article covering when you should get a Dental Associates for your practice.)

More than a few times a week I am asked to answer questions regarding when a Dental Practice should bring on a new Dental Assocatate. The questions come from dentists who need one (or think they need one) right away, as well as those just entertaining the idea for the future. There are obvious pros to adding an associate: more service for your patients, potentially more time off, someone to cover emergencies, etc. There are also some just as obvious cons: your patients may not like the new doctor, and you might worry about them making off with your patient base, etc. Thirteen years in private practice and close to as many as a trainer/consultant have taught me one hardbound rule: there is definitely a right and wrong way to go about this associate business.

If you are considering bringing on one or more dental associates (whether this is now or ten years from now), there are several things to consider:

1. When should you get an associate?
2. How would you structure compensation?
3. What’s the best way to find one?
4. What are the important points to cover when interviewing?
5. How will you integrate them into your practice?

I’ll start with number one: When should you get an associate?

This may be the most important question. This is also where the most errors are made. Let’s say you are doing moderately well, still have some openings in your schedule and get about 10 new patients per month. You decide to expand your hours and bring in an associate to become more productive. The reasoning seems sound – you are adding more hours and providing more treatment opportunities for your patients – but this rarely works. New patients don’t magically show up, the associate is unproductive and unhappy. You either a) let him or her move on or b) start moving work from your schedule to make the associate busier/happier. The net result is less profit and a problem, i.e “how do I keep my associate busy?”

In this scenario the office was in no position to justify adding an associate. As such, this begs the question: How do I know when the “right time” is? To answer this question, ask yourself the following:
1) Is your practice growing (or has it grown up to now and you just seem to have “maxxed out”)?
2) Are you scheduled efficiently?
3) Is your business profitable?
4) Is your schedule relatively full?

If you answered “Yes” to all of the above, now is probably a good time to add an associate.

I’ll give you this scenario: Your practice has rapidly (or steadily) expanded up to a point where you can’t take in more patients than you currently are. You are operating efficiently and the office is profitable. You just can’t see more people and things start to book out a couple of weeks in advance. Now is the time to add that associate to serve three purposes:

1) To provide faster and more efficient service to your patients,
2) To lighten your schedule so you can focus on the type of work you want to do and
3) To increase practice productivity.

If my practice was in the above situation, I would look at adding an associate – perhaps one to two days a week to start and roll from there.

From a practical standpoint, I would also look at how many charts I had. In my experience, handled effeciently, 1,000 charts can keep a doctor and hygienist productive. Maintaining a ratio of one doctor to one hygienist seems to also work well. If you are already have two full-time hygienists (who are booked), you may need an associate now. However, you also need to consider the other points above.

Business survival is inexorably connected to expansion. If the practice is growing (even a little bit), there will come a time when you can’t produce any more yourself and need an associate. The level of production that will require an associate will be based on your style of practice, fees, type of dentistry you do, and other factors.

How long is too long for them to wait? Although this is ultimately your decision, it shouldn’t be more than a couple of weeks. Too long of a wait is just not good service!
One of the questions asked above when deciding whether or not you should add an associate, asked about expansion. How much expansion do you need? To start, are you even getting enough new patients to support yourself? How many should you be getting? This depends in a large part on how you practice, but I’ll give you a basic formula to use:

1) Take your total number of active charts
2) Multiply this by 20%
3) Divide “b” by 12 (months in a year)
4) The figure from “c” above gives you the minimum number of new patients you should be getting on a monthly basis to maintain your practice’s health. Note that this is just to keep you going. You would definitely need to exceed this number to add an associate.

Example: Dr. Smith has 1200 active charts. 20% of this is 240. 240 divided by 12 is 20. To maintain a healthy practice, Dr. Smith should be getting at least 20 new patients each month.

Keep in mind this formula assumes a couple of conditions exist:
1) The new patients are fee-for-service.
2) The doctor has an acceptable skill level when it comes to treatment presentation and acceptance (which is reflected in production and collections).

If you want an associate, I recommend that you far exceed this 20% factor. This is where the “MGE New Patient Workshop” comes in handy – whether you want an associate or not. If you want more fee-for-service new patients to keep your practice healthy, to expand or to make it possible to add an associate the “New Patient Workshop” is the solution.

So you had that fantastic business idea, the one that’s going to be wildly successful and make you a fortune - even better, you actually did something about it and started your own business. Good for you! Not everyone gets that far. Most people sit and day dream about what they might do if only….

But you got over the biggest hurdle, that first step and you actually created something.

Well done - you did more than most. Now you have the day to day details of running your business so how do you keep going?

There are several things to look at here:

1. What are you good at and what do you enjoy doing? Make two lists - one of the jobs you like and one of the jobs you don’t do very well. Take the second list and have a look at what you might outsource or automate. Do you love doing those accounts or would your time be better spent in forward planning while your accountant does the sums? Must you personally reply to every enquiry or could you create a FAQ which you can post on your website and refer people to by autoresponder? In the early stages of your business you might find you don’t have the money to pay someone to do the jobs you hate. Could you swap skills to get the help you need?

2. Why are you doing this? You need to be motivated to start a business and keep it going. The best way to do this is to know what the effort is for. What really moves you to get up in the morning and do what you need to do even when you don’t really feel like it? Write your reasons down. Find pictures of that house, or that holiday and put them where you can see them.

3. How do you deal with those bumps in the road? Not everything you do will be perfect - sometimes things you’ve tried will be a disaster - it’s the way you react to problems that matters. If you curl up in a ball and give up at the first sign of failure you’d better not be in business. It’s all about attitude.

4. Have a plan and stick to it as far as possible but be prepared to be flexible and open minded. Sometimes the most unexpected opportunities come and you need to be ready to seize them. - as Joe Vitale says ‘Money likes speed’.

5. Get a mentor - learn from someone who has done it. Someone to bounce ideas off and who can encourage you when things get tough is invaluable.

And most importantly, never let anyone put you down and never be afraid of failure:

Collins’ team examined 1,435 companies to see which ones made substantial gains in profitability and sustained those improvements over 15 years or more. Since the 1970s, only 11 companies had risen from mediocrity to greatness and stayed there — topping many other prosperous firms that lacked the same staying power.

Of eight characteristics these companies shared, all held an unshakable adherence to becoming the best in the world at whatever they did. Each company committed to doing only those things and nothing else. That sometimes meant dropping their core businesses to pursue other things at which they could become the best in the world.

Collins and his team coined the term “hedgehog concept” to reflect a single-minded determination and focus that, similar to that of the hedgehog animal, attempts to do only one thing really well, such as curl up and roll. A hedgehog concept actually represents the intersection of three areas:

1) What you’re most passionate about
2) An understanding of what you could be the best at doing, and
3) A metric that drives your economic engine and helps you measure results.

Keep in mind that according to Collins, this concept is not a goal, strategy, or plan, but an understanding of what you can and can’t be the best at doing. Until you develop your hedgehog concept, you won’t know your true vision, mission, or purpose.

Next, Define Your “Business Success Criteria”

Do you have a crystal clear idea of the types of business undertakings that align with your gifts, talents, passions, and strengths? In that same context, have you thought about whether your business can be the very best in the world at doing those things?

I could make up a terrific story about this, but I won’t lie - I had avoided (as in postponed, side-stepped, procrastinated) having a board of directors until now. Frankly, I had visions of having a group of old, cranky, humorless men telling me what to do.

Of course I was just being lazy, too. I would rather be out making products and building a business than sitting around trying to make sense out of Excel files, charts and graphs, and essentially being bored to death in the process.

Our company, however, has reached the point where “proper governance” is important…even necessary. The “let’s do it because we all think it’s a really good idea” mentality had to go. We really needed to be able to show that all of our shareholders were represented in our decision making - and represented fairly.

So I asked my business mentor and close friend, who knows and understands our industry very well, to be the first member of the board of directors. Now let’s be clear - I didn’t ask him because he’s my “friend.” That would have shown very poor judgment, and frankly, friends don’t always make the best business advisors. I asked him because he’s already the one person who advises me on all “board-type” matters, anyway!

So imagine this: I felt like a “big grown-up boy” in long pants, carrying my briefcase filled with notes, reports, Excel printouts, etc., to my first board of directors meeting on Friday, February 15, 2008, at 2 PM ET.

If you are picturing a large dark paneled room with a long table, think again. Outside our “boardroom” were chickens, squirrels, birds, and other creatures - large and small, wild and domesticated. Inside the “boardroom” (besides the board members) were a dog (a.k.a. The Wolf), two cats (a.k.a. Puffy and Fluffy), and five children. Yes, we were in my friend’s home, gathered around his kitchen table.

Maybe someday we will meet in that dark-paneled room with a long table. But I don’t care how big my business gets - I hope we can continue to meet with the same “family feeling.” There was a certain calmness, almost a serenity, about the entire meeting. There was nothing stuffy or even formal, although we did follow the rules of a proper meeting.

So my first board of directors meeting started with a brief lesson about what exactly happens at board meetings! My friend and mentor gave a simple, five-minute explanation of what board meetings were all about…and in the process, he completely changed my preconceived ideas. That’s what I really want to share today.

What Do You Think Is Supposed To Happen At Board Meetings? - Company planning strategy?

- Hiring strategy?

- Financial planning?

No, no, and no. Those are the things that I THOUGHT were supposed to happen at a board meeting, but was I ever wrong. The things listed above are the territory covered by company management…not the board of directors.

The board of directors has exactly one responsibility, and that responsibility is…

GOVERNANCE

Just like a sovereign nation, each company has what they call “articles of incorporation.” These “articles” are actually the laws - or rules - that the management of the company must abide by.

So the whole purpose of the board of directors is just to make sure those laws are followed. The point is for the board to make sure the decisions that are made in the day-to-day operation of the company are really in the best financial interest of the shareholders.

Of course, not ALL of the decisions that are made by management are the right decisions - anyone can be wrong, it’s inevitable. But the decisions have to be made within the laws laid down in the articles of incorporation. They can’t be sneaky decisions, they can’t have malicious undertones, and they can’t be decisions that line the pockets of management at the expense of shareholders.

Here is just one example of the type of responsibility shouldered by the board of directors:

The board does not decide who is hired to fill a position. The board simply “empowers the management” to pursue that hire. It’s still management’s job to make the final decision about who is hired to fill the position. The board only acknowledges that they understand why the position has been created and filled.

The board of directors GOVERNS. It does not strategize.

So in the end, I didn’t need all those spreadsheet printouts and detailed notes. What I did need was exactly what I got - a lesson in how to think about shareholder value, while simultaneously running the company.
